Itachi Uchiha is considered to be one of the most fan-favorite characters, especially when his backstory was revealed to us. He was the brightest young ninja, with extraordinary natural talent right from his early childhood.

Itachi graduated from the Konoha Academy at a younger age than ever before, was the youngest Anbu member and became a Anbu captain before he even became a teenager. Furthermore, Itachi’s competence could be determined by the fact that he already awakened the Mankegyou Sharingan and he was also able to use a full body Susanoo.

But he had one of the saddest stories, as he was ordered to slaughter his entire clan by the villages higher-ups. Itachi was then burdened with the murders of his mother and father, but still, he showed what Sasuke meant to him as he let him live out his life.

The burden of killing his entire clan was tremendous, and he eventually became a rogue ninja, sealing the hatred in Sasuke so that his brother might grow strong and eventually kill him. This would have meant that Sasuke would have become the hero of Konohagakure although, it didn’t go according to plan.

He did die at the hands of Sasuke, but it wasn’t Sasuke who actually killed him as Itachi was never able to reach his full potential during the battle due to an unknown illness, which eventually became the very reason to his death. In actual fact, Itachi won the fight, however, the strain of the battle was too much for his body. So let’s discuss ‘What might have happened to Itachi and how he actually died?’

According to a fan theory, Itachi was suffering from Microscopic Polyangiitis. It is a rare form of the disease, which is the result of blood vessel inflammation (vasculitis), which can damage organ systems.

The most visible symptom of Itachi’s illness is hemoptysis as the symptoms include coughing, shortness of breath, chest pain, cardiac arrest etc. All of these were visible in Itachi during his fight.

Now, we know that overusing Mangekyou Sharingan can make the user go blind but in this case, the disease could have been responsible for speeding up the damage to his eyes. He took a variety of medication to keep him alive until he fought Sasuke, as he didn’t want to die before that.

Also, according to databooks, Itachi had very low stamina so he suffered from high-level of fatigue which could also have worsened or have been due to the disease. Itachi also looked pale and skinny which meant he lost few pounds of weight too.

Medical reports suggest that the age of the victim acquiring the disease is same as that of Itachi’s. But as it is not genetically communicable, his younger brother is safe, which is a relief to so many fans. This MPA can be caused by a trigger that sends the antibodies into overdrive.

So in this case, Itachi’s overuse of his Mangekyou Sharingan could have been the trigger, as using it too frequently might have sent a lot of antibodies to rebuild the damage caused by the use of Mangekyou Sharingan. But don’t worry about Sasuke, as he possesses the Eternal Mangekyou Sharingan now, which Itachi didn’t have.

In addition, we know that Itachi had a female interest in his clan, however, he was forced to kill her as well but possessed her with his genjutsu. In that genjutsu he showed her what their lives could have been, which included them having a family before killing her in the real world. Microscopic Polyangiitis is sexually transmitted and since no one else in Itachi’s family had this sickness, it’s possible that he was given it from her if they had a sexual relationship.

Now, it could have been possible that Itachi fell victim during his Chunin exams, however, his whereabouts during the time-skip is unknown. So this is likely how Itachi actually died, as it was not Sasuke who killed him.
